The biomechanics of the humeral head are influenced by several key factors: the size of the tear, the degree to which it retracts, the number of anchors used, and the varying density of the bone. Further research is vital to explore the intricacies of rotator cuff surgery and improve our knowledge regarding peri-anchor cyst formation. From a biomechanical perspective, the anchor configuration—connecting the tear to itself and other tears—and the tear type itself are essential elements. Further investigation into the biochemical properties of the anchor suture material is imperative. Developing a validated grading system for peri-anchor cysts would be beneficial.
The purpose of this systematic review is to examine the influence of varying exercise protocols on functional performance and pain experienced by elderly patients with substantial, non-repairable rotator cuff tears, as a conservative intervention. A PubMed-Medline, Cochrane Central, and Scopus literature search identified randomized controlled trials, prospective and retrospective cohort studies, and case series evaluating functional and pain outcomes after physical therapy in patients aged 65 or older with massive rotator cuff tears.
